If there is an incident in a game, a Supplementary Report must also be completed in addition to the Match Report.  Report forms can be downloaded
here.

An incident would include any of the following:

  • Serious injury
  • Send-off of a player, or ejection of a coach or spectator
  • Termination or abandonment of game (e.g., game suspension or termination due to lightning)
  • No show by a team
  • Unusual field conditions
  • Referee abuse or referee assault
  • Other events out of the ordinary.

In the case of a send-off or ejection, for travel and Cup games, the player or coach’s pass must be included with the report.

Robby or Mark are happy to help with anyone with filing reports.
